Referrals

Refer a patient in one call

Physicians, hospital and emergency department teams, discharge planners, therapists and employee assistance programs can refer adults to Longleaf Lodge for medical detox and residential treatment. Call admissions and say you are a referring provider. We take the clinical picture over the phone, check the patient's benefits and tell you whether the Lodge is the right level of care.

If it is not, we tell you on the call and help you find the setting that is, including partial hospitalization, intensive outpatient and outpatient care at Longleaf's own locations.

Making a referral

How to refer a patient to Longleaf Lodge

The Lodge treats adults with a substance use disorder, with or without a co-occurring mental health condition, who need more structure than outpatient care can give. Our clinical team confirms fit on every referral. The admission criteria, including who needs hospital or psychiatric care first, are on the admissions page.

  1. 01

    Call admissions

Call (844) 810-7340 and tell the admissions team you are making a professional referral.

  2. 02

    Share the clinical picture

We ask about substance use, withdrawal risk, medical and psychiatric history and current medications, and take insurance details for a benefits check.

  3. 03

Tell us how to keep you informed

Let us know who at your practice or agency should receive updates.

What to have ready when you call

  • The patient's name, date of birth and phone number, and their agreement to the referral
  • Substances used, how much, how often and the date of last use
  • Withdrawal history, including any seizures or delirium tremens
  • Current medications and doses
  • Medical conditions and any hospital discharge paperwork
  • Mental health diagnoses and current risk of self-harm
  • Insurance carrier and member ID

If you do not have all of this, call anyway. We can start with what you have.

After you refer

What happens after a residential treatment referral

  1. Benefits verification

Admissions verifies the patient's insurance benefits and any prior authorization the plan requires.

  2. Clinical review

to confirm detox, residential treatment or another level of care.

  3. An admission date

We tell you and the patient whether a bed is open and set a date.

  4. Arrival

The patient checks in at the Welcome House and begins in detox or moves into a cabin. With consent, we confirm the admission to you.
